- Certified court documents
- Individualized Education Program (IEP)
- 504 Accommodations Plan
- Foster/Group Home Placement
STEP 2 : FOLLOW - UP FROM RIVERA OFFICE
The Rivera office will review the online enrollment application and requested the documents.
The parent or legal guardian will be emailed a link to schedule an in-person appointment.
At that appointment, you must bring all original documents for verification.
ADDITIONAL INFORMATION
If your child is homeless or in foster care and you are unable to provide the documents listed above, visit the Rivera office directly.
